When Should I Arrive for my In-Office Appointment?
Please arrive 15 minutes prior to your appointment to allow for check-in. We ask that you be on time so that you will be able to keep your appointment. Late arrival may result in the cancellation or rescheduling of your appointment.
What Do I Need for my Appointment?
Please bring the following to your appointment:
- Current insurance card/government-issued ID (ex. drivers license, identification card).
- Current list of your medications, including dosage and directions, to each appointment.
What Forms of Payment do you Accept?
We accept cash, personal in-state checks, VISA, and MasterCard. There is a $40 service charge for returned checks.
What is your Cancellation Policy?
You will receive a reminder call the day before your appointment. If you need to change or cancel your appointment, please notify the office with at least 24 hours' notice. If you are scheduled for a procedure, 48 hours cancellation notice is required. We reserve your appointment time for you only. No-shows, last-minute cancellations, or late arrivals may result in a $50 cancellation fee. In order to protect the rights of other patients who are waiting to be scheduled, you may be asked to find another medical provider if you have two consecutive missed or canceled appointments.
What if I Need Help After Hours?
If you have a true medical emergency, call 911 or go to the Emergency Center of the nearest hospital. If you are experiencing an urgent issue that is not an emergency, please follow instructions on the voicemail system to page the doctor on call.
Will my Doctor Fill Out the Required Medical Forms?
At the discretion of the doctor, letters and forms requiring medical review and physician signature are subject to a $25 fee plus $5 per page/side. This fee must be paid prior to the forms being completed.
How do I Request my Medical Records?
In accordance with Texas law, StrideWoundCare requires written requests for the release of medical records. If you are in need of a copy of your medical records or if you wish to have a copy sent to another physician, we will be happy to prepare them. Please allow 15 business days for records requests to be processed. A fee may be assessed for personal copies.
